Through this model, residents are connected to preventative care, screenings, medical homes, and social support services—reducing avoidable emergency utilization and improving long-term health outcomes. With an initial $1.3 million supplemental request for 2026 and $2.0 million annually thereafter, this investment will scale an existing, results-driven program across Detroit, Flint, Lansing, and Saginaw. Statement of Need: Michigan faces rising chronic disease rates, increasing healthcare costs, and gaps in behavioral health access.
