This initiative addresses persistent gaps in healthcare access, chronic disease outcomes, and workforce shortages by embedding Community Health Workers (CHWs) within trusted community institutions. Through this model, residents are connected to preventative care, screenings, medical homes, and social support services—reducing avoidable emergency utilization and improving long-term health outcomes. With an initial $1.3 million supplemental request for 2026 and $2.0 million annually thereafter, this investment will scale an existing, results-driven program across Detroit, Flint, Lansing, and Saginaw. Statement of Need: Michigan faces rising chronic disease rates, increasing healthcare costs, and gaps in behavioral health access. Underserved communities face barriers such as transportation, health literacy, and trust in medical systems. Without investment in prevention and navigation, costs will continue to rise through Medicaid expenditures and avoidable hospitalizations.
